We have just learned of a potential issue with the back nut on DUI DELTA BCs with serial number # DU0507000 through DU0507149. It would have been purchased from DUI after July 1, 2007.

TITAN/CONSHELF DIN HANDWHEEL RETAINER

Aqua Lung was recently informed about the breakage, under pressure, of a TITAN/CONSHELF DIN HANDWHEEL RETAINER weakened during service by manual torque loads superior to the recommended values.

Aqua Lung asks all the owners of TITAN DIN regulators whose serial number is lower than 6062501 or TITAN/CONSHELF DIN adaptors that are marked 300 BAR MAX to return their regulator to their Aqua Lung authorized retailer for an upgrade.

The corrective action is replacement of the brass DIN HANDWHEEL RETAINER by another stainless steel DIN HANDWHEEL RETAINER marked 300 BAR MAXI.

This replacement is made under warranty.

All the TITAN DIN regulators whose serial number are above 6062501 and/or whose DIN handwheel retainer is marked 300 BAR MAXI are assembled with a stainless steel DIN handwheel retainer and are not affected by this recall.

PADI Nitrox 32 Tables

PADI Americas is recalling a single version of the English Imperial EANx32 Recreational Dive Planner used for calculating dive profiles. A printing error caused a series of numbers to be out of alignment, thus incorrect. The risk of injury from these errors is remote and no injuries have been reported.

All RDPs included in this recall bear the printing mark “Printed in the USA” located on the back of the table, upper right corner. The product number and production code are located on the lower right corner, also on the back of the table. The product number and production code affected by this recall are:

Product No. 60049 Ver 1.2 (Rev 02/03) XXXPDK7

The product code digits indicated by “X” may vary. The last four digits “PDK7” indicate a recalled table, irrespective of what the first three digits are.

As a double check, on Table 3, check the 110 foot row under Pressure Group G. The table should indicate a Residual Nitrogen Time (upper number in white) of 13 and an Actual Bottom Time (lower number in green) of 12. If this box is blank, you have a misprinted table.

We will be notifying consumers certified as PADI Enriched Air Divers after 17 January 2008 via the PADI website, e-mail and other means. They will be directed to stop using the recalled RDP immediately and bring them to any PADI Dive Centre/Resort or return them to PADI for a free replacement. If you replace one of these RDPs for a consumer, please notify your Sales Representative to receive your replacement product.

Apeks TX, ATX and XTX 2nd Stages

This is limited to those 2nd stages/octopus purchased before July 2008 that have not undergone an authorised annual service.
 
It has come to our attention that an incorrectly assembled 2nd stage has been purchased. If you have an Apeks regulator or octopus that has never had an authorised annual service, regardless of how recently purchased, please take the regulator/octopus to your Apeks dealer to be visually inspected.
All Apeks dealers and distributors have been made aware of this issue.
Do not dive with the regulator until it has been visually inspected. If it is not possible for you to return the regulator for inspection then please contact Apeks’ Customer Service Department for assistance. Apeks apologises for any inconvenience this may cause. Whites Recalls LP Hose

February 18th 2008

Name of Product:  Quick Connect LP Inflate Hose 29”, VLSITINF 32120

Manufacturer: Whites

Hazard:  Certain Dry suits that are equipped with inflation valves will also include the low pressure inflator hose.  There is a possibility that the low pressure hose quick connect will snap off of the hose. Shipment of hoses in question began on Sept. 1st, 2007.

Incidents/Injuries:  None reported.

Dealers will replace the Quick Connect LP Inflate Hose 29”(VLSITINF 32120) with our previous style STD Low Pressure Inflate
Hose (29”)(VLSITINF32500).  White’s Customer Service: 1-866-652-8556.

SeaLife Announces Important Technical Update

Name of Product:   SL961 Digital Pro Flash, SL168 - DC600 Elite (shipped from SeaLife commencing December 10, 2007), SL169 - DC600 Maxx Kit (shipped from SeaLife commencing December 10, 2007)

Hazard:  SeaLife recommends removing the spiral-sleeve protector or cables may break.

Incidents/Injuries:  Unknown.  It has been recognized by our Quality Control Department that the coiled sleeve which covers the cable inside the waterproof door can cause stress and possible damage to the cable connection. Repeated opening and closing of the door may weaken and break this cable. If the cable does break , the flash will still fire but the flash power adjustment function would not work.

In order to eliminate this possible breakage, we have removed the wrap from production and are suggesting that the wrap be removed from any unsold inventory in your store. Attached you will find a technical update that will also be available on our website at, www.sealife-cameras.com , that gives simple instructions on how and what to remove.

Product shipped from SeaLife as of January 21, 2008, will have been updated and can be identified by a red dot sticker on the outside of the packaging. We will continue to identify these updated items with these dots until February 15,2008.

AERIS Announces Voluntary Recall of AT-400 First Stage Regulators

Oceanic Voluntary Recall of FDX-10 and CDX-5 First Stages

Name of Product:  FDX-10 and CDX-5 first stage regulators shipped to Oceanic Retailers from the factory between May 1, 2006 and November 15, 2006. Other model DX type First Stages, such as the CDX, DXi, DX3, DX4, and TDX5, that received annual service between May 1, 2006, and October 22, 2007, may also be affected.

Aqua Lung America Recalls Surface Observation Signal (S.O.S.)

Name of Product: Surface Observation Signal (S.O.S.)

Manufacturer: Aqua Lung

Hazard: Some elbows have exhibited signs of degradation leading to cracking and/or breakage.  If this were to occur, air may not successfully transfer from the BC to the safety tube, thus preventing the tube from inflating.  Furthermore, if a diver were inverted, making the attachment point the “high spot,” air could also escape from the BC.  The leak may prevent the BC from “fully inflating.” Although the BC will still hold air when vertical, some air will escape at the bottom and the BC will provide less buoyancy.

Incidents/Injuries: To date, there have not been any reported accidents or injuries.

M&J Engineering Recalls Swivels for Scuba Diving Masks Due to Drowning Hazard

Name of Product: AGA Swivels for Scuba Diving Masks

Units: About 1,500

Manufacturer: M&J Engineering, of Poway, Calif.

Hazard: The swivel, which is attached to a diving mask, could separate while diving. This will result in a sudden loss of the diver’s air supply, causing the diver to engage in emergency ascent. This poses a risk of decompression sickness due to rapid ascent or drowning.

Incidents/Injuries: M&J Engineering has received one report of swivels separating from the masks while diving. No injuries have been reported.

Description: This recall involves all SW-3000 2nd stage 360-degree swivels for scuba diving masks manufactured between January 2004 and February 2005. The swivel has the date stamped on its body, below the patent number. Swivels that are already repaired have an arrow stamped on the top right corner of the swivel body.

Sold at: Diving retail stores nationwide between January 2004 and February 2005 for about $60.

Remedy: Consumers should immediately stop using the scuba diving masks with the swivel attached and contact M&J Engineering to receive a free repair.

Innovative Scuba Concepts Recalls Diving Regulator Components Due to Drowning Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: Swivel for a Scuba Regulator

Units: About 170

Importer: Innovative Scuba Concepts Inc., of Colorado Springs, Colo.

Hazard: The swivel, which is attached to a diving regulator, could separate while diving. This will result in a complete and sudden loss of the diver’s air supply, causing the diver to engage in emergency ascent procedures. This poses a risk of decompression sickness due to rapid ascent, and air embolism or drowning if the diver panics or the emergency ascent procedure fails.

Incidents/Injuries: The firm has received one report where the swivel separated during a dive and caused the diver to ascend using the buddy breathing technique. No injury was reported.

Description: The recall involves the HO110 Swivels sold as an aftermarket regulator component. There are no markings such as a date or production code embossed on the unit. Contact Innovative Scuba Concepts for information on where the recalled units were sold.

Sold at: Specialty retail dive stores nationwide from January 2006 through March 2007 for about $40. They also could have been installed by a dive shop regulator technician.

Manufactured in: Taiwan

Remedy: Consumers should stop using regulators with the swivel attached and contact the dive store where purchased for a refund.

Pelagic Recalls Digital Dive Computers Due to Decompression Hazard

Name of product: Oceanic and AERIS Digital Dive Computers

Units: About 2,800

Manufacturer: Pelagic Pressure Systems, of San Leandro, Calif.

Hazard: When performing a switch from one gas to another during a dive, the dive computer’s display will lock up and not return to the main dive screen that displays dive times. This can cause divers to enter decompression unknowingly or the diver could ascend prematurely, resulting in decompression sickness.

Description and Models: The recall involves Oceanic-brand ATOM 2.0 dive computers with serial numbers 1 through 2,079 (Revisions 2E, 3A, and 3B) and AERIS-brand EPIC dive computers with serial numbers 1 through 712 (Revision 1A), which can be accessed and viewed on the computer’s display. Also, the serial number and date of manufacture are printed on the bottom of the unit (Oceanic ATOM 2.0 from August 23 to November 23, 2006, and AERIS EPIC from October 18 to November 14, 2006). This recall does not include any other Oceanic or AERIS brand dive computers.

Hazard: A manufacturing error could cause the main housing of some regulators to change shape over time, which could cause the cover and diaphragm to become dislodged. If this occurs, air flow will be interrupted and the regulators will no longer function, posing a drowning hazard to users.

Suunto D6 and D9 dive computers

July 2006

Suunto Oy has identified a software bug in the D9 and D6 diving instruments. The software bug may cause the D9 and D6 to incorrectly track dive time on rare occasions, potentially causing a risk to the diver. For further information please read more here. Halcyon Stainless Steel Power Inflator Recall

May 2004

Halcyon is recalling all stainless steel power inflators. The only units affected can be easily identified by their stainless buttons and started shipping on all Halcyon BCs after Oct 21, 2003. This recall is in response to problems experienced by some Halcyon customers... Typically, the problem expresses itself as a slow leak into the BC bladder. The cause of the leak relates to small imperfections within the machining of the stainless air barrel of some inflators. Not all of the inflators exhibit the machining flaws... In order to reduce the inconvenience and to return your BC to operation as quickly as possible, we are shipping new inflators to your local Halcyon dealer. You may bring your inflator to your dealer, trade it for a new unit, and have it fitted to your buoyancy compensator. UWATEC SMART PRO and SMART COM dive computers

 UWATEC is conducting a voluntary recall of the Smart PRO and Smart COM dive computers. A software programming error can render audio and some visual alert signals inoperable, and under certain conditions, the screen freezes. You can determine if you have one of these units by looking for the "Smart" name on the front of the computers. For safety reasons, we ask that you stop using the dive computer  immediately.

Zeagle 1st Stage Regulator Recall

Dec 2003

Zeagle Systems Inc. is recalling 931 first-stage scuba regulators sold between January 20, 2003, and May 19, 2003. If you have one of these regulators, he said, "it is essential to take it to your dealer for immediate repair. Do not attempt to dive with this regulator again until the regulator has been inspected and by an authorized Zeagle dealer." The first-stage regulators affected are the following models: * FlatHead Six, s/n F001391 to F001476; * 50D, s/n B003315 to B003652; * DS-IV, s/n A001352 to A001571; * Envoy, s/n C001117 to C001403. Serial numbers are stamped on each first-stage body. Zeagle regulators that have been serviced since January 20, 2003, may also be affected. These first-stage regulators may have a defective high-pressure seat that could cause the first stage to over-pressurize. If this occurs, the air supply to the diver could be interrupted, making it impossible for the diver to breathe from the regulator. This type of failure is most likely to occur upon initial pressurization of the regulator and is unlikely to occur underwater. If you own one of these regulators, take it to your authorized Zeagle dealer for a free inspection and repair.
